Community Health Worker Training Pathway (Bulk Pricing)


A community health worker (CHW) is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. In this pathway, you will be guided by our expert instructors on strategies to enhance your CHW skills.

The Patient Navigation and Community Health Worker Training Program is offering a discounted price of $1,500 for enrollment in the CHW Training Pathway. This is a $500 savings compared to paying for each class individually.  This pathway includes 4 instructor-led courses that will qualify you to take the state of Colorado assessment and be listed as a credentialed CHW.

This pathway includes:

  • Level 1: Health Navigation Fundamentals
  • Level 2: Care Coordination
  • Motivational Interviewing Basics
  • Community Health Worker Fundamentals

These additional self-paced courses are required for credentialing (available at no cost on the PNCT e-learning website):

  • Intro to the Healthcare System
  • Preventive Healthcare 101
  • Intro to Chronic Disease
  • Health Insurance Basics
  • Trauma-Informed Care
  • Cultural Navigation

Please note, you will register for each course individually as they are scheduled. You may choose to participate in either in-person courses, online courses or a mix of both.
