Level 2 Care Coordination Online

Designed for health navigators and community health workers, this course offers a hands-on journey into the heart of team-based care. You’ll build essential skills to boost client engagement, streamline workflows, and champion patient safety. What to Expect

This course is instructor-led with one live learning session (LLS) per week. You will complete weekly homework before attending each LLS. The live sessions will be interactive with both large and small group activities. We ask that you participate in the live learning sessions by video to connect with your peers. The estimated time commitment is 3-4 hours per week.

5 Live Learning Sessions

Sessions will be held on the following Thursdays, between November 6 and December 11, 10 - 11:30 AM MT:

  • November 6: Course Orientation
  • November 13: LLS 1
  • November 20: LLS 2
  • NO CLASS on Thanksgiving Week
  • December 4: LLS 3
  • December 11: LLS 4

Course Access

The course materials will be available 1 week before the course orientation. You will receive additional instructions from the course support person at that time. Make sure you sign in prior to your first live session to complete the required activities and ensure you know how to connect with your camera. Successful Completion

To earn a Certificate of Completion, you must complete the following:

  • Attend and fully participate in 3 of the 4 Live Learning Sessions
  • Complete 80% of online course activities.
  • Complete the post-course evaluation.

Learning Objectives

By the end of this course you will be able to:

  1. Describe the components of high-quality care coordination.
  2. Communicate effectively with team members and patients to achieve the aims of care coordination.
  3. Efficiently manage a patient panel.
  4. Use evidence-based strategies to support patient self-management goals.
  5. Measure the effectiveness of team-based care coordination at the individual patient and population levels.

  • If you are unable to attend the course, you must cancel your registration at least 10 days before the first day of training in order to receive a refund.
  • Cancellations are subject to a $75 cancellation fee.
