Level 2 Care Coordination Online


Healthcare delivery can be very fragmented, which often results in poor health outcomes, patient dissatisfaction, and higher healthcare costs. Coordinated care contributes to patient-centered, high-quality care. This course focuses on the role of the health navigator or community health worker in team-based care coordination. You will be guided step-by-step to learn the basic skills and advanced strategies to enhance team-based care and client engagement for better health outcomes, patient safety, and workflow efficiency.

What to Expect

This course is instructor-led with one live learning session (LLS) per week. You will complete weekly homework before attending each LLS. The live sessions will be interactive with both large and small group activities. We ask that you participate in the live learning sessions by video to connect with your peers. The estimated time commitment is 3-4 hours per week.

Sessions will be held on the following Thursdays, between June 26 and July 31, 10 - 11:30 AM MT:

  • June 26: Course Orientation
  • NO CLASS on July 3
  • July 10: LLS 1
  • July 17: LLS 2
  • July 24: LLS 3
  • July 31: LLS 4

Course Access

The course materials will be available 1 week before the course orientation. You will receive additional instructions from the course support person at that time. Make sure you sign in prior to your first live session to complete the required activities and ensure you know how to connect with your camera. Successful Completion

To earn a Certificate of Completion, you must complete the following:

  • Attend 3 of the 4 Live Learning Sessions
  • Complete 80% of homework assignments.
  • Complete the post-course evaluation.

  1. Attribute all members of the care team with specific roles and functions in high-quality care coordination.
  2. Communicate effectively with team members and patients to achieve the aims of care coordination.
  3. Efficiently manage a patient panel with (and without) large-data management tools.
  4. Use evidence-based strategies to support patient self-management goals.
  5. Measure the effectiveness of team-based care coordination at the individual patient and population levels.
