Level 2 Care Coordination IN-PERSON COURSE


Healthcare delivery can be very fragmented, which often results in poor health outcomes, patient dissatisfaction, and higher healthcare costs. Coordinated care contributes to patient-centered, high-quality care. This course focuses on the role of the health navigator or community health worker in team-based care coordination. You will be guided step-by-step to learn the basic skills and advanced strategies to enhance team-based care and client engagement for better health outcomes, patient safety, and workflow efficiency.

What to Expect

This course is 1.5 days in person and will be highly interactive with small group activities. No prework required. It is recommended to complete Level 1: Health Navigation Fundamentals before Level 2: Care Coordination.

  • Day 1: October 7, 2025, 9:00 AM - 4:30 PM MT.
  • Day 2: October 8, 2025, 9:00 AM - 12:00 PM MT.

Successful Completion

To earn a Certificate of Completion, you must complete the following:

  • Attend and fully participate in the 1.5-day course.
  • Complete the post-course evaluation.
